UP Free health Insurance Scheme 2017 Yojana for Free Medical Treatment Smart Card To BPL Rashan Card Holder Hospital list UP Cashless health Scheme 2017 is available.

UP Free Health Insurance Scheme 2017 is Swasthya Bima Yojana For BPL Rashan Card Holder to give Free Medical Treatment like Obama care. To Sign up for UP Free Health Insurance Yojana & National Health Insurance Scheme you must have valid BPL Rashan Card issued by Kotedar of your Gram Panchayat, Village, Town and Cities. UP cashless Health Treatment scheme starting date is February 2017. Its Main Objectives is to provide BPL families access to Best medical care treatment of illness including hospitalization, surgery and therapies through Super specialty health care under registered hospitals list.

UP Health Smart Card Yojana: Swasthya Bima Card will be Distributed to BPL Card holder follow the objectives for free medical treatment scheme. Your Photograph, Name, Town and District will be written on Health Smart card.

Benefit: BPL (Below Poverty Level)families of both rural and urban areas of the State are eligible to avail the benefits of the scheme, Ration Card issued by the Food & Civil Supplies and Consumer affairs of Uttar Pradesh.

Limitations of the cashless health Scheme: Benefit is restricted to only five members of a BPL family in a year. Sum assured will be Rs.1.50 lakh on a family floater basis per year. Additional buffer of Rs.50,000/- per yea

Age limit: No age limit for availing the benefit of the Cash health Scheme

402 Diseases Covered: Neurological Diseases, Cancer, and Burns ((not applicable to motor vehicle accident cases), Radiation Oncology etc.

How To Apply for Health Insurance Card: The regular Monthly Health Camps are held at the District & Taluk Level. The following are scheduled dates. Beneficiaries are identified on the basis of following Manners

i) Permanent BPL cards with photograph issued by Food and Civil Supplies Department

ii) Temporary BPL cards with seal and attestation by the jurisdictional Tahsildar.

Hospitals List: Private Nursing Homes, Hospitals and Government Institutions equipped with super speciality health care.
